You can also download this form from the internet — you will need your APN#, which you could obtain here Scroll down and click the "Accept" under "Proprietary Information." This will take you to a screen where you can enter your physical address, click "Submit" to get your APN#. Click on "Assessor's Home Page" (upper left corner), choose "exemption" found under "Forms" on the left column. Click "BOE-266, ASSR 515, ASSR 517" to download a two-page pdf file.
